What is Home Health Care?
We can provide an intermittent service or a combination of intermittent services in your home under the direction of a physician. Working with your doctor, our qualified staff will plan, coordinate, and provide care tailored to your needs, tapering visits as you improve and stabilize, or increase visits if your condition worsens.
Who is eligible for Home Health services?
Admission can only be made under the direction of a physician and is based upon the patient's identified care needs. homebound status and the type of services required.
How do I request Home Health Services?
You and your doctor can call and request our services. If you are currently in the hospital, the hospital discharge planner can make a referral for you.
What is Hospice Care?
Hospice provides care, comfort and support to those persons nearing the end of life's journey. Hospice will help you make decisions about how and where you want to spend the rest of your life. Hospice treats you, not the condition or disease. The focus is comfort, not cure. Hospice considers not only you, but your family. Through ever advancing technology, pain and symptom control will enable you to live life as fully and comfortable as possible. Hospice emphasizes quality, rather than length of life.
What is the criteria for Hospice Admission?
Admission to our hospice program is made upon recommendation of your physician based on your needs. Normally, appropriate candidates for Hospice are patients who have a life expectancy of six (6) months or less if the illness runs its normal course, who desire palliative treatment, who want to stay at home as long as possible and who have a primary caregiver.
